Clinicians at CPC use Simple Practice. This is a HIPAA-compliant and secure patient portal platform. You can access your portal on a desktop or on your phone.
Existing patients: please log-in to the patient portal to access your chart.
New Patients: Please log-in to your secure client portal account using the information sent by your provider in order to complete all required forms prior to your first appointment.
